29 CFR 1910.119(e)(6): The process hazard analysis was not updated and revalidated every five years: a) Facility located at the Radford Army Ammunition Plant: On or about March 7, 2025, the employer did not ensure the process hazard analysis for the MACS Black Powder Loading process was revalidated at least every five years to assure it represented the current process and hazards continued to be identified, evaluated and measures were taken to mitigate the identified hazards. The failure to update and revalidate the Process Hazard Analysis (PHA) when changes occur, or at least every five years, poses explosion risks to employees due to the lack of identification and evaluation of hazards associated with the process.

29 CFR 1910.119(f)(1)(ii): The employer's written operating procedures did not address the requirements for the operating limits listed in 29 CFR 1910.119(f)(1)(ii)(A) and (B): a) Facility located at the Radford Army Ammunition Plant, MACS Line: On or about March 7, 2025, the employer failed to develop and implement clear procedures that identified the operational limits of the process, including temperature and humidity levels, as well as the potential consequences of not adhering to those levels. Operating the process outside the defined temperature and humidity range may lead to static electricity, posing fire and explosion risks to employees. b) Facility located at the Radford Army Ammunition Plant, MACS Line On or about March 7, 2025, the employer failed to develop and implement clear procedures for managing the consequences of not operating the vacuum or for operating it outside the specified psi levels. Additionally, they did not detail the actions to take if the desired psi cannot be met or is exceeded. Failure to use the vacuum appropriately may result in the buildup of dust, which poses fire and explosion risks to employees. c) Facility located at the Radford Army Ammunition Plant, MACS Line On or about March 7, 2025, the employer failed to develop and implement clear procedures for addressing the consequences associated with storing chemicals in strong sunlight. Explosives kept in direct sunlight may detonate, putting employees at risk of fire and explosion hazards.
