29 CFR 1926.451(a)(1):Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it:Note: Exceptions are provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section. Location: Jobsite at 299 Beacon Street, Boston, MA On or about March 5, 2025 and times prior, workers were exposed to collapse and fall hazards where a scaffold, which had been wrapped with flame retardant weather protection, had not been adequately constructed to withstand the wind loads transmitted to it and collapsed under the wind load.

29 CFR 1926.452(c)(4):End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur: Location: Jobsite at 299 Beacon Street, Boston, MA On or about March 5, 2025 and times prior, workers were exposed to collapse and fall hazards where a scaffold, which had been enveloped in weather protection was lacking pins connecting the end tubes of the scaffold. High wind conditions caused the bracing to fail and uplift to occur causing the scaffold to collapse.
