29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On or about January 23, 2025, the employer did not report within 24 hours the amputation injury of an employee who received a partial fingertip amputation injury in the workplace on or about January 22, 2025.  The employer was notified the injury resulted in an amputation injury on or about January 22, 2025, and OSHA was not notified of the amputation injury until on or about February 28, 2025, via telephone to the OSHA Cleveland Area Office.
