29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about March 10, 2025, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.  Z Construction #1 Inc. was previously cited for a violation of this occupational safety and health standard 1926.102 (a)(1), which was contained in OSHA inspection number 1754404, citation number 1, item number 1(b) and was affirmed as a final order on October 29, 2024, with respect to a workplace located at 505 W. Golf Road, Mount Prospect, IL 60056.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. On or about March 10, 2025, at the above addressed jobsite, employees performing residential roofing operations, nine (9) feet, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Z Construction #1 Inc. was previously cited for a violation of its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1754404, citation number 2, item number 1 and was affirmed as a final order on October 29, 2024, with respect to a workplace located at 505 W Golf Road, Mount Prospect, IL 60056.  Z Construction #1 Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1717714, citation number 1, item number 1 and was affirmed as a final order on February 5, 2024, with respect to a workplace located at 812 N Goodwin Dr., Park Ridge, IL 60068.

On or about March 10, 2025, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
