29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about March 10, 2025, at a jobsite located at 30906 Kramer Dr. Waterford, WI. 53185, employees conducting roofing activities on a residential structure to include tear-off,  and shingle installation on a 10:12 pitch roof were not protected from falling to the lower level by a personal fall arrest system, or other means of fall protection. This exposed employees to a fall hazard.  The Roofing Pro's LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number1599691, citation number 1, item number 2, and was affirmed as a final order on December 15,2022, with respect to a workplace located at W258S4866 Red Clover Dr. Waukesha WI. 53186.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, or were not secured:  (a) On or about March 10, 2025, at a jobsite located at 30906 Kramer Dr. Waterford, WI. 53185, the portable extension ladder being used to access the roof of the structure did not extend at least three feet above the upper landing area and was not secured. This exposed employees accessing and egressing the roof to a fall hazard.  The Roofing Pro's LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard  1926.1053(b)(1), which was contained in OSHA inspection number1599691, citation number 1, item number 4, and was affirmed as a final order on December 15,2022, with respect to a workplace located at W258S4866 Red Clover Dr. Waukesha WI. 53186.
