On March 7, 2025, an employee performing tray inspections suffered a finger amputation injury while attempting to unjam cookies where the belt tension rollers meet the conveyor belt on Line 7 - Doboy 00693 - Wrapper Feeder and Conveyor. This condition exposed employees to an in-running nip-point and amputation hazard.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a)Hearthside Food Solutions LLC dba Maker's Pride at One Devil's Food Way, North Sioux City, SD, 57049:  On or about March 7, 2025, the employer did not ensure all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy source were performed. Employees did not deenergize and lockout hazardous energy sources on the Doboy 00693 - Wrapper Feeder and Conveyor located on Line 7 while attempting to unjam cookies. While performing this task, the applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from in-running nip points and amputation hazards. On March 7, 2025, an employee performing tray inspections suffered a finger amputation injury while unjamming cookies where the belt tension rollers meet the conveyor belt on Line 7 - Doboy 00693 - Wrapper Feeder and Conveyor. Performing maintenance work on the energized Doboy Wrapper Feeder and Conveyor machine exposed employees to an in-running nip-point and amputation hazard.
