29 CFR  1910.147(c)(4)(i):  Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  132 N Elm St, Westfield, MA:  On and before March 12, 2025, the employer did not require employees to lock out or unplug the electrical glue pump machines while unclogging them.  Employees were exposed to the internal moving parts and the unguarded pump shafts when they removed the in-take hoses and checked for suction on the operating pumps.  On March 9, 2025, an employee received a fingertip amputation injury while attempting to unclog an operating glue pump.
