29 CFR  1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  a) On or before March 11, 2025, an employee was exposed to health and safety hazards associated with chemicals such as but not limited to caustic soda, citric acid, Sodium Hypochlorite 10.0%, and chlorine bleach in that they had not been properly trained by the employer on the location of SDS.

29 CFR  1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:   a)  On or before March 11, 2025, an employee was exposed to respiratory irritation due to potential inhalation of citric acid.  The tight-fitting negative pressure half-face respirator provided to an employee use was not stored in a manner to protect it from damage or becoming dirty.
