OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a) Fill Plant: On or about March 11, 2025, the employer failed to provide route drivers with sufficient training and information regarding pressure relief valves to prevent struck by hazards while transferring liquid nitrogen from bulk storage tanks to transport tanks.

The event occurred on March 7, 2025 at approximately 7:55 AM and OSHA was not notified until March 10, 2025 at 4:30 PM.
