On or about March 11, 2025, at an establishment located at 406 Winton Street, in Wausau, Wisconsin; in the Main Machining Area/Weld Area the employer failed to ensure that employees did not utilize the top step and top cap of the Kpro Keller step ladder exposing the employee to a fall hazard of approximately 46 inches.  An employee used the step ladder to perform tasks and gain access to the table for the Giddings & Lewis horizontal boring machine.

In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  On or about March 11, 2025, at an establishment located at 406 Winton Street, in Wausau, Wisconsin; in the Main Machining / Welding area, the Gidding & Lewis horizontal boring machine was missing the cover on the electrical terminal strip box which was used to power the rotary drill exposing employees to electrical shock hazards.
