Employees accessed and worked from the frame scaffold to install new bricks.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Location: 1116 Blake Ave, Brooklyn, NY a) On or about 3/10/2025, employees worked from a tubular welded frame scaffold without using any form of fall protection or being protected by a guardrail system. The employees working on installing new bricks were exposed to falls of approximately 15 feet to the ground level.

Site: 1116 Blake Ave, Brooklyn, NY 11208 a) On or about 3/10/2025, employees performed work from tubular welded frame scaffold that did not have guardrail systems installed on the outrigger platform. The end of the working platform on the scaffold was not guarded.
