29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about March 10, 2025, the employer did not ensure that the competent person was conducting frequent and regular inspections of the job site to ensure appropriate fall protection was being worn and fall protection systems were installed properly, and that ladders were installed properly.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about March 10, 2025, two employees performing tear off work on the south side of an existing residential roof with a 12:12 pitch, were using body belts in leu of full body harnesses and fall arrest system. This exposed employees to a fall hazard of approximately 25 feet from roof to the ground below.  (b) On or about March 10, 2025, an employee performing tear off work on the north side of an existing residential roof with a 12:12 pitched, was using a body belt in leu of full body harness and fall arrest system. This exposed employees to a fall hazard of approximately 25 feet from roof to the ground below.

29 CFR  1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) On or about March 10, 2025, three employees performing tear off work on the south side and north side of an existing residential roof with a 12:12 pitched, were using body belts and connected the lines to the hip d-rings.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about March 10, 2025, employees performing tear off work on an existing residential home, used two (2) Aluminum extension ladders to access the roof. The extension ladder from the balcony to the roof was not erected three (3) feet above the upper landing or secured to the structure.

29 CFR  1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement.   (a) On or about March 10, 2025, employees performing tear off work on an existing residential home, used an Aluminum extension ladder from the balcony to the roof that was on the slope of the gable, and which was not on a stable surface to prevent accidental displacement, exposing employees to a fall hazard.
