29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure: a) Job site, 1303 South Braddock Avenue, Braddock, PA: On or about March 11, 2025 during roofing work, the employer did not ensure employees used appropriate fall protection systems/devices and were exposed to fall hazards of up to approximately 10 feet in that some employees used no fall protection at all and one employee tied a rope to a chimney and then around his waist as a means to prevent falling off the edge of the roof.

29 CFR 1926.20(b)(2): The employer did not institute a program of frequent and regular inspections of the job sites, materials, and equipment to be made by competent person: a) Job site, 1303 South Braddock Avenue, Braddock, PA: On or about March 11, 2025, during roofing work, the employer did not ensure that a system of hazard identification and correction was implemented at the job site. Deficiencies noted during the inspection included, but were not limited to, not using fall protection on the roof, nails in boards/shingles point upwards in the walking/working areas and working near a live electrical service drop with no insulative protection.
