Jobsite, 1005 Pemberton St. Pittsburgh, PA 15216: On or about March 11, 2025, employees were working on a residential roof height up to 11 feet 17 inches above the ground and were not utilizing fall protection.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.) Jobsite, 1005 Pemberton St. Pittsburgh, PA 15216: On or about March 11, 2025, the employer did not have in writing any training certifications to verify that employees were trained on when and how to use fall protection. Employees were exposed to an approximately 11 feet 17 inches fall hazard while replacing a residential roof.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a.) Jobsite, 1005 Pemberton St. Pittsburgh, PA 15216: On or about March 11, 2025, employees were using a portable ladder to access the residential roof. The ladder side rails did not extend at least 3 feet above the upper landing surface. Employees using the ladder to access the roof were exposed to a fall hazard of approximately 11 feet and 17 inches to the ground level.
