29 CFR  1926.451(g)(1)(vi):  Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).  On or about March 11, 2025, at an exterior building wall, employees were performing overhand bricklaying work activities at heights greater than ten (10) feet from a scaffold that did not have a guardrail system installed along all sides and the employees were not in use of fall protection.

29 CFR  1926.451(h)(1):Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  On or about March 11, 2025, at an exterior building wall, employees were performing bricklaying work activities from the second level of a scaffold that did not have toeboards installed while employees working in areas below the scaffold.
