29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation. On or about March 11, 2025, on the corner of N.W. 17th and Kingsbury, an employee was performing pipeline installation work activities in an excavation with accumulated water at the base of the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about March 11, 2025, on the corner of N.W. 17th and Kingsbury, an employee was performing pipeline installation wok activities in an excavation that was more than five(5) feet in depth and was not protected by adequate protective systems. Southwest Water Works LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1622729 citation number 1, item number 1, and was affirmed as a final order on April 3, 2023, with a final abatement date of July 3, 2023, with respect to a workplace hazard at 8208 N. Kelley Ave., Oklahoma City, OK, 73131.
