29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Roof, 6210 University Boulevard E, Cottondale, AL 35453: On or about March 12, 2025, the employer exposed employees to fall hazards, in that the employer did not ensure that the employees were wearing fall protection, installed guardrails, or implemented a warning line system around the unprotected sides and edges of the 2/12 pitch commercial roof while performing Thermoplastic Olefin (TPO) roofing material installation activities at heights 6 feet and greater to the ground below.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: (a) Roof, 6210 University Boulevard E, Cottondale, AL 35453: On or about March 12, 2025, the employer exposed employees to fall hazards in construction, in that the employer did not prepare a written certification record for fall protection training that contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training, as employees installed Thermoplastic Olefin (TPO) roofing material on a commercial 2/12 pitch roof at a height of approximately 13 feet, 10-inches to the ground below.
