Location: Warehouse area of M. Cutone Mushroom Company, Inc.  On or about March 8, 2025, an employee of M. Cutone Mushroom Company, Inc. was exposed to fall hazards while being lifted on a wooden pallet, on the forks of a forklift, in order to retrieve a case of bottles from the top level of a racking system.
