On a new commercial construction, at a temporary jobsite located at 880 N Spring Garden Avenue, Deland, FL 32720: On or about March 12, 2025, two employees were exposed to an over 25-foot fall from elevation to lower surface hazard while conducting metal roof work, without wearing full body harness or other types of fall protection.

On a new commercial construction, at a temporary jobsite located at 880 N Spring Garden Avenue, Deland, FL 32720: On or about March 12, 2025, one employee was exposed to an over 20-foot fall from elevation to lower surface hazard while operating an aerial lift, conducting metal roof work, without wearing a full body harness and untied to the lift's basket.
