On or about March 11, 2025, at an excavation located on S. Monticello near W. Admiral Drive in Stillwater, Oklahoma, employees were performing work in an excavation that was more than 4 feet deep without the use of a safe means of egress.

29 CFR 1926.652(g)(2): Excavations of earth material to a level no greater than 2 feet (.61m) below the bottom of a shield was permitted. On or about March 11, 2025, at an excavation located on S. Monticello near W. Admiral Drive in Stillwater, Oklahoma, employees were performing work in an excavation that was more than 4 feet deep and the sheild was positioned more than two (2) feet above the excavation bottom.
