29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about March 10, 2025, during the installation of framing material at the job site located at 3975 Matisse Cir, Pepper Pike Ohio 44124; during the installation of framing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 21 feet.

29 CFR  1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about March 10, 2025, during the installation of framing material at the job site located at 3975 Matisse Cir, Pepper Pike Ohio 44124; the employer did not ensure employees working on a residential structure were protected from possible danger of eye injury from impact, or from flying objects from the employees installing siding material with air driven nail guns and cutting material with circular saws.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 10, 2025, during the installation of framing material at the job site located at 3975 Matisse Cir, Pepper Pike Ohio 44124; the employer did not ensure employees working on a roof of a single-family home were protected from a fall of varying height approximately 12.9 to 21 feet above the ground by utilizing a fall protection system.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about March 10, 2025, during the installation of framing material at the job site located at 3975 Matisse Cir, Pepper Pike Ohio 44124; during the installation of framing material on a new construction residential structure, the employer did not ensure that employees utilized portable ladders that were extended 3 feet above the upper landing surface. The foreman was on site and utilized the portable ladder to gain access/egress to the working surface of the roof which was 12.9 feet above the ground.
