29 CFR  1910.28(b)(3)(i)  The employer did not ensure that each employee was protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):  a) Northeast Corner of Roof: On or about March 10, 2025 and at times prior thereto, the employer exposed employees to falls through skylights with a fall height of approximately 28 feet, in that the employer did not ensure that fall protection was used by employees while servicing air conditioning units on the roof of a customer's building.
