29 CFR 1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  Site Wide - On or about March 12, 2025, employees using pneumatic nail guns to install shingles were exposed to struck-by hazards from projectiles because they were not wearing safety glasses or other eye and face protection equipment. 29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) Main Roof - On or about March 12, 2025, employees working on a residential roof were exposed to a fall hazard of approximately thirty and a half (30.5) feet because they were using climbing harnesses that did not anchor in the back near shoulder level or above the wearer's head in lieu of personal fall arrest system. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Main Roof - On or about March 12, 2025, employees using an extension ladder were exposed to a fall hazard of approximately thirty and a half (30.5) feet in that the ladder was not installed such that the side rails extended at least three (3) feet above the roof edge.
