Crabtree Corners, Jacksonville, FL: On or about March 13, 2025, the employer exposed employees to fall hazards, in that a 12-foot Gorilla extension ladder was used to access the top of the structure and did not extend three feet above the eave as required.

Crabtree Corners, Jacksonville, FL: On or about March 13, 2025, the employer exposed employees to fall hazard, in that fall protection was not utilized by employees performing re-roofing operations. The Commonwealth Roofing Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1488907, citation number 1, item number 1 and was affirmed as a final order on May 12, 2021, with respect to a workplace located at 8828 Ivey Road, Jacksonville, Florida 32216.
