29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. At 317 Base Leg Drive, Edgewater, Florida: On or about March 12, 2025, in the Fabrication Area of the shop, the employer exposed employees to an amputation hazard, in that the Hydraulic Tube Bending Machine Model Hines 200NC(M116) did not have a guard properly installed to provide protection of the point of operation and/or nip points. b. At 317 Base Leg Drive, Edgewater, Florida: On or about March 12, 2025, in the Fabrication Area of the shop, the employer exposed employees to an amputation hazard, in that the Leonard Tubemaster Model 2CP(M162) did not have a guard properly installed to provide protection of the point of operation and/or nip points. c. At 317 Base Leg Drive, Edgewater, Florida: On or about March 12, 2025, in the Fabrication Area of the shop, the employer exposed employees to an amputation hazard, in that the PHI Model 2CPV(M244) did not have a guard properly installed to provide protection of the point of operation and/or nip points.
