29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:   a) On December 12, 2024, and at times prior thereto, employees were exposed to falls of approximately 11.7-feet to approximately 45-feet while performing roofing activities.  FKA Construction, LLC was previously cited for a substantially similar violation of occupational safety and health standards (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 1682761, citation number 1, item number 1 and was affirmed as a final order on 12/28/2023, with respect to a workplace located at 1702 N Greenhead Dr., Meridian, ID 83642.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support:  (a)  On December 12, 2024, employees were exposed to a fall hazard of approximately 11.7-feet while using a ladder to access the roof of a new construction commercial building.  The top end of the ladder was resting against the eve of the roof and did not extend above the landing surface.  (b) On December 12, 2024, the employer did not ensure its subcontractor protected its employees who were exposed to a fall hazard of approximately 11.7-feet while using a ladder to access the roof of a new construction commercial building. The ladder extended approximately 2.5-feet past the landing surface.  FKA Construction, LLC was previously cited for a substantially similar violation of occupational safety and health standards (29 CFR 1926.1053(b)(1)), which was contained in OSHA inspection number 1682761, citation number 1, item number 2 and was affirmed as a final order on 12/28/2023, with respect to a workplace located at 1702 N Greenhead Dr., Meridian, ID 83642.
