29 CFR  1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:   At the jobsite located at 21810 W 53rd Street, Shawnee, Kansas, employees engaged in residential roofing activities using a pneumatic nail gun were exposed to struck by hazards in that eye protection was not utilized.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  At the jobsite located at 21810 W. 53rd Street, Shawnee, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 20 feet from the eave of the roof to the ground. Fall protection was not utilized.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Employees performing residential roofing work were exposed to fall hazards of approximately 20 feet in that employees had not received fall protection training on how to minimize these hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the jobsite located at 21810 W 53rd Street, Shawnee, Kansas, employees using a portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 20 feet from the roof to the ground. The side rails of the portable extension ladder did not extend at least 3 feet above the landing surface of the roof.  The side rails extended approximately 2 feet.
