29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) Jobsite: 9125 Academy Rd, Philadelphia, PA - On or about March 13, 2025, the employer did not ensure employees utilized personal protective equipment when handling chemicals. Employees handled a Quikrete product containing Portland Cement without the use of hand protection.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Jobsite: 9125 Academy Rd, Philadelphia, PA - On or about March 13, 2025, the employer did not provide hazardous chemical training for employees engaged in a CMU wall construction. Employees were not trained on the hazardous chemicals, including but not limited to Portland Cement contained in the Quikrete Portland Lime & Sand Mortar Type S .
