29 CFR 1910.215(a)(1): Machine guarding. Abrasive wheels shall be used only on machines provided with safety guards as defined in the following paragraphs of this section. On or about March 12, 2025, at 215 Louisiana 19, SLAUGHTER, LA 7077, employees in the main shop area used a bench grinder without proper guards while performing grinding operations.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about March 12, 2025, employees engaged in machining operations were exposed to electrical shock hazards because the employer failed to visually inspect the flexible cord used to power a fan with a damaged power cord.
