At the Kitchen cabinet workshop, located at 155-B Old Daytona Road, in Deland, FL: On or about March 13, 2025, the employer exposed employees to the hazard of being struck-by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured: Shop Areas - PVC plastic piping was used in exposed above ground locations to transport compressed air at 110 psi exposing employees to struck-by hazards from shards of PVC if the pipe ruptures or explodes.
