29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about March 13, 2025, the employer failed to develop and implement a Control of Hazardous Energy Program.  Employees are exposed to struck by and/or caught in between hazards when performing tasks, such as but not limited to, clearing of jams on the Granulator machine that has hazardous energies, such as but not limited to, electrical and tension.

29 CFR  1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about March 13, 2025, the employer failed to ensure the Granulator machine was locked and tagged out to control the hazardous energies of electrical and tension when employees are clearing jams.  Employees are exposed to struck by and/or caught in between hazards when performing this task.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:    On or about March 13, 2025, the employer failed to ensure that employees tasked with clearing jams on the Granulator machine were trained as authorized to perform lock out tag out prior to clearing the jam.  Employees are exposed to struck by and caught in between hazards when performing this task and not locking and tagging out the machine which has hazardous energies, such as but not limited to, electrical and tension.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:   On or about March 13, 2025, the employer failed to report a multi finger amputation that occurred on March 5, 2025, within the required 24 hours.  An employee sustained a multi finger amputation while manually clearing a jam on the granulator machine.
