29 CFR  1910.1200(f)(6)(ii):  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.   a. On or about March 13, 2025, in the facility, spray bottle used in the COP room was not labeled with the appropriate hazard warning of the chemical(s) they contained. Employee exposure included but was not limited to Rubout AFCO 5758 and Lance AFCO 5255.

29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard")  a)	On or about March 13, 2025, at the above addressed jobsite, the employer allowed employees to use half mask tight fitting respirators during daily duties.  The employer did not provide the employees using respirators with the information contained in "Appendix D" as required by this standard.
