29 CFR  1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:  (a)Mundo Construction and Roofing at 310 Casey Tibbs Street, Fort Pierre, SD 57532:  On or about and at times prior to March 13, 2025, the employer did not ensure that frequent and regular inspections of the job site, materials, and equipment were being conducted by a competent person.  This condition exposed employees to various hazards including, fall, ladder, and forklift hazards.

29 CFR  1926.760(a)(1):Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:  (a)Mundo Construction and Roofing at 310 Casey Tibbs Street, Fort Pierre, SD 57532:  On or about and at times prior to March 13, 2025, the employer did not ensure employees were protected from fall hazards while replacing corrugated steel roofing decking material on a wooden pole-to-truss pole barn. This condition exposed employees to fall hazards greater than 15 feet.

This condition exposed employees to fall hazards greater than 15 feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)Mundo Construction and Roofing at 310 Casey Tibbs Street, Fort Pierre, SD 57532:  On or about and at times prior to March 13, 2025, the employer did not ensure the Werner extension ladder being used on site to access the roof of the commercial structure for steel roofing activities was extended at least 3 feet above the upper landing surface being accessed.  This condition exposed employees to a fall hazard  of 12 feet.

This condition exposed employees to fall hazards in excess of 12 feet.
