29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet above or below the point of access: a) On March 14, 2025, company employees were climbing a folding aluminum ladder to access a pump-jack scaffold platform up to approximately 18.6 feet above the ground where the ladder did not reach the scaffolding platform. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a) On February 21, 2025, employees working from a pump-jack scaffold on the east side of the structure were exposed to the hazard of falling approximately 16.7 feet . a) On February 21, 2025, employees working from a pump-jack scaffold on the west side of the structure were exposed to the hazard of falling approximately 18.6 feet. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
