29 CFR  1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:  (a) On March 14, 2025, the employer did not ensure that the Exit sign over the door in battery repair location nearest the main lobby was illuminated.  (b) On March 14, 2025, the employer did not ensure that the Exit sign over the door in the rear (east) portion of the warehouse was illuminated.  (c) On March 14, 2024, the employer did not ensure that the Exit signs along the aisleways in the warehouse were illuminated.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about March 13, 2024, at the above addressed jobsite, the employer failed to train employees on the operation of powered industrial trucks.  Employees were exposed to struck by hazards, fall hazards and improper use such the uneven loading of pallets on forks of powered industrial truck.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   (a) Shop warehouse area - On or about March 14, 2025, employees were exposed to the corrosive chemical hazards of sulfuric acid while lifting and wrapping corroded batteries by hand without the benefit of effective hazard communication and training.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.303(g)(1)(i)(B):The width of working space in front of the electric equipment shall be the width of the equipment or 762 mm (30 in.), whichever is greater. In all cases, the working space shall permit at least a 90-degree opening of equipment doors or hinged panels.  On or about March 14, 2025, on the west side of the building nearest the dock door of the warehouse, electrical equipment was blocked to prevent access.  Certification of abatement is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
