General-duty citation text
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees On or about March 12, 2025, on the east side of the dock, a laborer for Associated Terminals added a tag line to an existing tag line to guide a barge cover from the pier to the barge. A crane was hoisting the barge cover. The exposed employee added the second tag line, leaving excess slack on the ground. The exposed employee's foot was caught up by the slack of the tag line as the crane began to hoist the cover, resulting in the excess rope tightening around the employee's foot, lifting him from the dock to the barge, injuring his left foot. As a result, the employee's left foot had to be surgically amputated.
