29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. On or about March 6, 2025, Commercial Kitchen Services Inc. located at 808 Hanley Industrial Court in St. Louis, Missouri, did not provide safety training to include ladder safety and fall prevention program to the injured employee who fell from a ladder at the worksite, resulting in injury from a fall at a height of 18 feet.

29 CFR 1910.49(d)(2)(ii): The employer did not ensure that personal fall arrest systems are rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level. A free fall may be more than 6 feet (1.8 m) provided the employer can demonstrate the manufacturer designed the system to allow a free fall of more than 6 feet and tested the system to ensure a maximum arresting force of 1,800 pounds (8 kN) is not exceeded. On or about March 6, 2025, Commercial Kitchen Services, Inc. located at 808 Hanley Industrial Court in St. Louis, Missouri, did not provide fall prevention training and fall equipment to employee #1 who fell from a ladder at the jobsite, resulting to a falling injury at a height of 18 feet.
