29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about March 17, 2025, on the east side of the commercial building employees who installed roofing material on the dumpster Roof worked from heights of more than 6-feet without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about March 17, 2025, on the east side of the of the commercial building, employees who installed roofing material used an extension ladder that did not have the side rails extended 3-feet above the upper landing surface.
