29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  Location: 19 Vine St, Weymouth, MA  On or about January 28, 2025, frequent and regular inspections were not adequately conducted to identify and correct worker exposure to fall hazards where fall protection was not utilized and to identify and correct worker exposure to fall hazards and crushed-by hazards when workers were conducting framing activities. The house being framed lacked sufficient lateral bracing and shear resistance during construction and collapsed during construction resulting in a fatality and injuries to several workers.

29 CFR  1926.501(a)(2):The employer did not determine if the walking/working surfaces on which its employees were to work had the strength and structural integrity to support employees safely:  Location: 19 Vine St, Weymouth, MA- 2nd floor and roof  On January 28, 2025, workers were exposed to fall hazards and crushed-by hazards when a two-unit, two-story house collapsed due to insufficient lateral bracing and shear resistance. The manner in which the house was being constructed lacked the strength and structural integrity to safely support workers.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location: 19 Vine St, Weymouth, MA  On January 28, 2025, a worker was sheathing a roof at heights of approximately 25 feet above ground level where fall protection was not used.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Location: 19 Vine St, Weymouth, MA  On January 28, 2025, a worker fell to the ground when the house being framed collapsed while he was sheathing a roof where fall protection was not used . The worker had been exposed to fall hazards of up to 25 feet to the ground prior to the collapse and did not have fall protection training  to recognize and minimize fall hazards.
