On or about March 7, 2025, the employer did not ensure that an employee utilized the documented energy control procedure for the Line 1 Filler when performing service and maintenance covered activities. As a result, the employee was exposed to pinch point hazards when performing cleaning washdown activities on the Line 1 Filler.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. On or about March 7, 2025, line operators were not trained as authorized employees on the recognition of all hazardous energy sources, their magnitudes, as well as the methods and means necessary for energy isolation and control on the dip filling lines. As a result, an employee was exposed to pinch point hazards at the Line 1 Filler while performing changeover activities.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: as required by 29 CFR 1910.147(d)(1) through (d)(6). a. On or about March 7, 2025, an employee was exposed to pinch point hazards when performing service and maintenance covered activities on the Line 1 Filler. The employer failed to implement energy control application steps as the machine was not shut down or turned off, [per the 1910.147(d)(2) requirements] when performing cleaning washdown activities on the Line 1 Filler. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.
