29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about March 18, 2025, at 4912 Grace Road, North Olmsted, OH 44070, the employer failed to ensure employees were protected from a fall of approximately 16 feet while utilizing a portable ladder to gain access to the upper landing surface in that the ladder was only set up at the first rung while climbing onto the unguarded frame.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about March 18, 2025 at 4912 Grace Road, North Olmsted, OH 44070, the employer failed to ensure employees were protected from a fall of approximately 16 feet from a roof with a 4:12 pitch in that no fall protective systems/devices were utilized while employees were performing sheeting operations. Buckeye Framing and Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1735691, citation number 01, item number 001 and was affirmed as a final order on July 1, 2024, with respect to a workplace located at 374 Shetland Court, Highland Heights, OH 44143.
