29 CFR 1910.179(n)(2)(ii):The load was not attached to the load block hook by means of slings or other approved devices: a) On or about 03/17/2025 and at times prior, in the Annealing Bay (BFS-Bar Finishing Storage East) area, employees were exposed to struck-by and crushed-by hazards when approved lifting devices were not used. Employees, moving bundles of steel bars weighing approximately 1,500 - 3,200 pounds from a staging area onto an entry roll table of the annealing furnace, attached an overhead crane's two-leg chain sling directly to carbon tie wire that is used to wrap the bundles of steel bars. The carbon tie wire was not rated or approved for lifting. ABATEMENT DOCUMENTATION REQUIRED.

Synthetic web slings have been in continuous use for approximately 6 months and the employer did not perform and document slings inspections. ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.1001(j)(3)(i):The building owner, facility owner, or employer did not determine the presence, location, and quantity of ACM or PACM at the worksite and did not exercise due diligence in complying with these requirements to inform employers and employees about the presence and location of asbestos-containing material (ACM) and presumed asbestos containing material (PACM): a) On or about 03/18/2025, in the Annealing Bay (BFS-Bar Finishing Storage East) Area; a large overhead pipe which runs the length of the Annealing Bay had damage to the insulation. Insulation was not tested to determine quantity of ACM (asbestos containing material), and or PACM (presumed asbestos containing material), including Thermal system insulation (TSI). ABATEMENT DOCUMENTATION REQUIRED.
