Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) On or about March 18, 2025, at a jobsite located at 1301 E St Paul Ave, Waukesha, WI 53186, employees conducting roofing activities (roof installation) on a 2:12 pitch roof of the commercial structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the roof of the structure exceeding 25 feet to the ground below.
