On or about March 13, 2025, in the new service shop, employees worked on a scaffold while erecting a new wall with CMU blocks and the space between the platform and the uprights were more than 1 inch wide.

29 CFR  1926.451(b)(5)(ii):The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.  On or about March 13, 2025, in the new service shop, employees walked and worked on scaffolds where the platform extended 2 feet beyond where the support was installed.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  On or about March 13, 2025, in the new service shop, workers used the cross braces of a scaffold to reach their work area to do masonry work.

For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  On or about March 13, 2025, in the new service shop, employees were walking and working on the second tier of a scaffold that did not have toe boards installed while CMU blocks were stored on the third tier.
