OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to the hazard of being struck by a heat exchanger tip-over: On or about March 15, 2025, employees were in the tip-over area of the heat exchanger's bonnet channel while using a plasma cutter to remove the stainless steel head from the bonnet channel.
