Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked. a. On or about March 19, 2025, in the warehouse, the employer did not ensure aisles and passageways where powered industrial vehicles travel was kept free from obstructions and impediments. Employees retrieve items using a ladder that protrudes into a narrow passage frequently used by forklifts and pallet jacks, and where permanent aisles are not appropriately marked or maintained. Thereby exposing employees to struck-by and fall hazards.
