On or about March 19, 2025, in the warehouse, employees were required to operate Komatsu powered industrial vehicles equipped with front end attachments that were not provided with a secondary load plate decal that identifies use with the extension boom. Employees were thereby exposed to injuries associated with struck-by hazards.
