29 CFR 1926.451(b)(1):Each platform on all working levels was not fully planked or decked between front uprights and the guardrail supports: (a) On or about February 20, 2025 and at times prior to that date, Total-Western, Inc., as the controlling contractor, allowed employees of Acuren Inspection to work from a scaffold, which was not fully planked or decked, exposing employees to fall hazards in excess of 10 feet.
