On or about March 20, 2025, and times prior, the employer exposed employees to cave-in/entrapment hazards in that the employer failed to conduct daily inspections of the excavation prior to the beginning of the day and as needed to determine if hazards are present.

On about March 20, 2025, and times prior, the employer exposed employees to fall hazards of up to 13-feet in that the ladder provided for egress from the excavation/trench was not extended at least 3-feet above the landing area located on the top of the trench/excavation.
