Location: Worksite On or about March 19, 2025, employees were exposed to fall hazards of over 40 feet while working from the fabricated frame scaffolding to install copper downspouts and elbows, where the employer did not ensure that a competent person made frequent and regular inspections of the jobsite, materials, and equipment.
